Guidelines for Financial Funding
(These guidelines based on info from each members application form)
- Prior to any funding we physically reach out to each applicant to discuss the reason to get a better understanding of their needs.
- All conversations 100% confidential and the only form of identity to the applicant is an applicant ID number.
- All members who wish to receive funding must be a member of MY MS FAMILY by becoming a member through the membership process. 1 Year membership $10, 2 year membership $18
Membership Requirement
All applicants must be members of MY MS FAMILY to receive funding.
1-Year Membership: $10
2-Year Membership: $18
- House cleaning fixed rate $75 per month
- Any purchase or requested funding that is at the $200 level if funding is available we cover 100%. Yearly funding is $1800
- Any applicant who has not filed to ODSP or ADP we will help with the application by directing them to their OT (Occupational Therapist) to do so. - The amount of funding for equipment after the applicant has reached out to their insurance company, ODSP, ADP then what ever the outstanding balance that is the responsibility of the applicant we will do our best to cover 25% to 50% of the cost depending on the financial ability of the applicant. Advisory Board will need to deal on a one to one basis.
- All decisions are based on funds available and under certain circumstances a waiting list will need to be implemented and the advisory board will decide the order of applicant on the urgency of the applicant. Need of equipment or service for immediate use supersedes an upgrade of equipment and service.
